In the Acknowledgments of this new edition, the authors list a number of ‘pals’ who have died since the first edition was published in 1995. Included are Greg van Wyngarden and Stewart Taylor. I have not seen this reported anywhere. If correct this is sad news. I was in touch with Greg very recently.
